Unity Hospice was created to offer a holistic and compassionate choice for end of life care. We are dedicated to the belief that everyone should have an opportunity to actively participate in their plan of care, having not only their physical needs met, but their concerns, desires, and hopes heard.
Unity Hospice is the first in New England to provide exclusive hospice services to residents of Harmony Homes. Unity provides concierge, 40 hours/week tailored hospice care to your loved one.
We will work together with you and your loved ones to provide a supportive, team approach to end of life care that emphasizes communication & compassion. Under the direction of our skilled Medical Director & Director of Patient Services, our team undergoes extensive and ongoing training in the needs of people who are in the final stages of life. We will work together to help our patients and their family through these passages with dignity and compassion, allowing for peace & healing at the final phase of life.

Eligibility & Referrals
A physician must certify that you or your loved one has a life limiting illness. A referral can be made by a patient, family member, or physician. Once on hospice, patients must be routinely reassessed to ensure they continue to qualify. The patient or decision making family member may revoke hospice at any time.

How is Hospice paid for? Hospice is a Medicare benefit, meaning there is no out of pocket cost associated with care or services provided by our team.
How do I know if my loved one qualifies for hospice? Together with our Medical Director, we utilize a variety of clinical information to determine if the time is right for your loved one to join our family. We encourage early intervention when it comes to end of life needs.
What will the Medicare benefit cover? The Medicare Hospice benefit covers visits from nurses and physicians, counseling team, volunteers, durable medical equipment, prescriptions related to your illness, and so much more.
What are the benefits of hospice? Hospice helps families with the transition from seeking curative means of living to focusing on quality of life for our loved one's last season with us. We focus our time on life review, symptom management, and ensuring a passing that is dignified and honorable.
Does hospice mean I am giving up? Not at all! It means that you have recognized this season of your life requires a care team that can help you to the finish line, in your own time and with optimized comfort levels. Hospice does not hasten death, rather, it invites healing and closure throughout the journey to end of life.
